ROCK HALL — Free sports physicals are being offered at the Rock Hall Elementary School Based Health Center this summer for students.
The clinics will be provided by Choptank Community Health on Mondays and Thursdays. Call 667-377-2004 or 410-479-5772 to make an appointment.
Students must bring their completed physical form to the appointment, as well as enroll with Choptank Community Health either online or by form.
Parents and guardians do not need to be present if they have given verbal permission for their child to be seen by the provider.
Rock Hall Elementary School is located at 21203 W. Sharp St., Rock Hall.
